Link below will take you to the Evaluation Form, a copy will be sent to you (the reviewer) and to Cara.

Once the review form is completed a copy will go to Cara and she will provide you with a final copy to obtain signatures and discuss with employees.

Supervisors are to only complete evaluations for 90 Days Evaluation & Quarter 1, 2, and 3.

1st Quarter (Jan., Feb., Mar.) Due by: April 15th

2nd Quarter (Apr., May, Jun.) Due by: July 15th

3rd Quarter (Jul., Aug., Sep.) Due by: Oct 15th

4th Quarter (Yearly, done by Personnel) 

Absence Form

If you are going to be absent for your shift please fill out the form below. 

Vacation Cash-In Request

Use this form to request cash-in for unused Vacation Time up to 40 hours. Request must be received by December 5th of each year.
